Midle name is being lost

Description

When entering a middle name and choosing "Add more" the middle name entered is lost. Also, if I enter a middle name and go to the View Summary page and then back using Edit the middle name is lost.

Tested suing the Add more buttons and also advanced to the next page and went back to edit. The middle name is now preserved.
