Clarify "one file" structure of main submission

Description

In Submit Step 3 (ETD Upload), there is a statement on the screen regarding the structure of the main submission that says "Also, if each chapter of your ETD is a separate file, you must combine these files into a single document.". This statement is incomplete because more than the chapters need to be in the single document. Please clarify the statement so the student is aware that all main sections of the ETD need to be in the one document: preliminary pages (such as copyright page, title page, abstract, acknowledgments, table of contents), main text (usually in chapters), and any references, footnotes, citations, bibliography, or curriculum vitae. Occasionally appendices may be included in the primary document, or they may be located in supplementary files that are identified in Submit Step 4.
